FATAL THREE-VEHICLE COLLISION UNDER INVESTIGATION IN LOMBARDY

(SOUTH ELMSLEY TOWNSHIP, ON) - The Ontario Provincial Police (OPP) is investigating a fatal three-vehicle collision on Highway 15 in Lombardy between County Road 1 and County Road 5.
On Friday, May 29, 2026, at approximately 5:00 pm, members of the Leeds County Detachment of the OPP responded to a report of a head-on collision involving two cars and an ambulance near Otter Lake Park. The ambulance was returning to base and had no patients onboard.
The driver of one of the vehicles, a 34-year-old male from Elizabethtown-Kitley, was pronounced deceased at the scene. A young child in that vehicle was airlifted to a trauma centre by Ornge air ambulance with life-threatening injuries. The driver and passenger of the ambulance were taken to hospital with serious injuries. The driver and sole occupant of the third vehicle sustained minor injuries and was transported to hospital.
Leeds Thousand Islands Fire Service, Leeds Grenville Paramedic Service, and OPP Technical Collision Investigators and Reconstructionist assisted Leeds County OPP in response to the incident. Highway 15 was closed for several hours and has since reopened.
